I feel the fact that he made a thread about the issue answers your question.

They've been more and more frequent lately. I think the fact that Revision3 advertises itself as a "real", studio quality production house and then leaves stupid mistakes like this in speaks for itself.
I dont even know what media offline gaps are
but if its removing mess ups then dont do it - I like to see stuff that wasnt sosposts be in the show in there, but not a lot of retakes or whatever just strange things that werent planed that are cool

We work to try and avoid Media Offline gaps (and they are relatively rare) it happens every once in a while when more than one editor is working on the same episode. Diggnation has a lot of motion elements & cam switches. We can't always review each 40-50min export to check them for media errors before it has to go live on the site.
My biggest gripe is that Final Cut even LETS us export a file with "media offline". Who wants a big red error message in their export? Not me. The problem could get solved in the next update of FCP.
Meantime, we'll keep an eye out- and every time we miss one... finish your drink!

Now that's a way to answer a question. I personally haven't noticed too many of them when watching Diggnation and when I do I'm not bothered by them...shit happens.
